in the "Look Up A License" feature of this website. This secure
program is updated daily. Paper licenses are not secure and are only accurate
at the time they are printed. Paper licenses and certificates are easy to forge
and are used by impostors for fraud and identity theft; these are serious
issues that threaten our mission of public protection. In the rare event an
employer will not accept the Look Up A License verification, you may request a paper verification form.
Passport-style Photograph Required
A properly formatted passport-style photo is
required to be submitted with all initial licensure and certification applications. Please
click here for examples of passport-style photos. If you have a digital photo close to the example, you can use this
on-line tool to upload and crop your photo to meet the passport-style standard. (Adobe Flash Player 10 or later required)

Initial Nursing Assistant Certification
