On-Line Initial Licensure & Certification Applications

​Effective January 27, 2012:  Please be advised that the disclosure of your Social Security Number is mandatory in order to process your application.
Any license or certificate application(s) received at the Maryland Board of Nursing without a Social Security Number (SSN) WILL NOT be processed.  Applications without a SSN are incomplete.
The Board is required by federal and Maryland law to collect this information for the following purposes:
  • Verification of identity with respect to final adverse actions related to your license or certificate (42 U.S.C. § 1320a-7e(b))
  • Administration of the Child Support Enforcement Program (Md. Family Law Code Ann., § 10-119.3)
  • Identification by the Maryland Department of Assessments and Taxation of new businesses in Maryland (Md. Health Occ. Code Ann., § 1-210)
    Primary source verification of certification status is found in the "Look Up A License" feature of this website. This secure program is updated daily. Paper licenses are not secure and are only accurate at the time they are printed. Paper licenses and certificates are easy to forge and are used by impostors for fraud and identity theft; these are serious issues that threaten our mission of public protection. In the rare event an employer will not accept the Look Up A License verification, you may request a paper verification form.
Passport-style Photograph Required

A properly formatted passport-style photo is required to be submitted with all initial licensure and certification applications. Please click here for examples of passport-style photos. If you have a digital photo close to the example, you can use this on-line tool to upload and crop your photo to meet the passport-style standard. (Adobe Flash Player 10 or later required)
Supported Web Browsers

The Initial Application web sites work best with Microsoft's Internet Explorer Browser. Mozilla Firefox, Google Chrome & Apple Safari browsers are NOT supported and may NOT work properly! 
Initial Nursing Assistant Certification

First-time Nursing Assistant applicants may now submit applications for initial certification via the Board's website. 
Instructors who need to complete the class list for their students may click here to submit the Class Confirmation list.
Registered Nurse / Licensed Practical Nurse

Nurses who are licensed in another state and have never held a Maryland license may apply for licensure by endorsement. Endorsement applicants must complete a Background Check before completing the on-line application.
If you have EVER held a Maryland license and it is currently listed as Inactive, Non-Renewed, or Invalid, you must request a renewal packet from MBON.nurselicenserenewal@maryland.gov.  If you have an active license in another state but have never held a Maryland license, continue with the endorsement process. ALL FEES ARE NON-REFUNDABLE AND NON-TRANSFERABLE.
RNs and LPNs graduating from nursing programs without a clinical component can now apply for an endorsement license in Maryland - More information
NOTE - If you DO NOT currently reside in Maryland, please send an e-mail to mdbon.nets@maryland.gov with your name and current mailing address to receive the required Background Check forms prior to applying on-line. Maryland residents ONLY may use this form​ to complete the Background Check.

4. Complete the online RN/LPN Endorsement Application
The Maryland Board of Nursing requires applicants for Temporary Licenses to use a Permanent Mailing Address when completing the application for licensure. This is especially important for licensees who are not going to reside in Maryland for an extended period of time.
***If your program is not on the list of Approved Out of State Nursing Programs, please provide the Board with the Name of the Nursing Program, Dean or Director of Nursing, and the email address to MDBON.NETS@maryland.gov. ***

You will need:
a digital passport style photograph in the PNG or JPG file format
the 12 digit number from under the barcode on the purple fingerprint card or the tracking number on the LIvescan receipt
a Visa or Mastercard credit or debit card ($100 for the permanent license and $40 if you need a temp)
Active license from another US State.
