Please be advised that the Board cannot fulfill requests for mailing lists at this time. Due to recent changes to the Public Information Act, the Board can now disclose only the business address of a licensee or certificate holder. However, the Board′s licensure database does not currently have the capacity to store a business address in addition to a home address in a manner that would enable the Board to extract business addresses into list form at this time. Accordingly, requests for a licensee or certificate holder′s business address must be submitted on an individualized basis.​
