Community Health Worker Advisory Committee

​Applications to serve on the State Community Health Worker Advisory Committee are being accepted through August 1, 2018. 

Applicants may apply for the following Advisory Committee seats:

  • Nine Community Health Workers (CHW)
  • One RN with experience in community health
  • One licensed social worker with experience in community health
  • A representative of a CHW training organization
  • A representative of a community-based employer of CHWs
  • One member of the public who is familiar with the services of CHWs

A copy of the legislation (SB 163) which created the Advisory Committee is attached.  The Committee members are appointed by the Governor with the advice and consent of the Senate.

To be considered for appointment, interested individuals must submit a formal  application by August 1 via the Governor's Appointments Office (GAO) website link.
 
Please note that when completing the application, applicants are prompted to choose a State Agency/Commission from a drop down menu -  the Committee is listed under "Health Worker".
